To participate in the Cardiovascular Prevention and Rehabilitation Program, you will need a referral from your doctor.
Ask your doctor for a referral to our program if you have any of the following conditions:
- heart attack
- heart surgery
- angioplasty
- heart failure
- other heart conditions, such as angina or pacemaker
- other vascular conditions, such as stroke or peripheral arterial disease
- high risk of heart disease, such as family history of heart disease, high cholesterol, high blood pressure, or diabetes
When making a referral, your doctor will include:
- a letter of referral indicating the reason for cardiac rehabilitation
- clinical notes relevant to the referral
- tests done, if relevant
